Candidates should have a degree in Physical Therapy, an active VA license, and ideally 2 years of experience. Qualifications

• Minimum of 2 years of physical therapy experience, home health preferred.

Responsibilities

• Provide home-based physical therapy to patients recovering from illness or injury.
• Conduct patient assessments and develop individualized treatment plans.
• Monitor patient progress and modify care plans as needed.
• Assist patients with exercises to improve mobility and manage pain.
• Educate patients on therapy goals, exercises, and self-care techniques.
• Coach family members on safely assisting with exercises.
• Collaborate with interdisciplinary team members.
• Maintain accurate documentation of patient progress.
