Job summary

Metro Physical & Aquatic Therapy is seeking a motivated in-home Physical Therapist (per diem) to work in Suffolk County, including Melville/Dix Hills. Pay is $70-$90 per visit with flexible schedule and both 1099 and W-2 options. All levels welcome; mentorship provided for documentation and compliance. Travel to patients' homes within the assigned geographic area is required.

Qualifications

• Physical Therapist program graduate required.
• Licensed PT in NY or eligible to practice in NY.
• Home health experience preferred; mentor provided for docs/compliance.
• Must be willing to travel to patients' homes within the territory.

Responsibilities

• Accept and treat patients within your geographic area.
• Evaluate home-based PT needs and create individualized plans.
• Develop care goals with physician, patient, and family.
• Plan and implement customized therapy to restore function.
• Instruct family, PTAs, HHAs, and caregivers on techniques.
• Provide timely, accurate documentation for each visit.
• Communicate with patients and team, esp. on discharge planning.
• Deliver care in line with agency policies and standards.
