Job summary

Qualifications

• Requires degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR certification.

Responsibilities

• Perform initial and ongoing mobility and functional assessments.
• Develop and implement written plans of care with goals and outcomes.
• Deliver physical therapy services per physician orders.
• Document timely and accurately to support payer reviews and discharge planning.
• Coordinate care with healthcare team and educate patients and caregivers.
• Supervise PTA/HHA staff and complete select OASIS assessments where allowed.
