Job summary
Traditional Home Health Care Pediatric Division is seeking a Licensed Practical Nurse to provide one-to-one nursing care in home and school settings. The role includes administering medications, implementing care plans, and coordinating with clinicians to ensure high-quality patient outcomes.

Qualifications
- Active LPN license required.
- Experience in home health care or pediatric settings preferred.
- Flexible scheduling including days, evenings, or overnight shifts.
Responsibilities
- One to One patient care in home/school environment.
- Implement and document the plan of care to ensure quality and continuity of care for patient.
- Administration of prescribed medication, treatments and therapies.
- Patient assessments; coordination of care.
- Reports any changes in the client condition to Clinical Manager and physician.
- Health promotion, teaching, and training of family members.
