Job summary

Aspire Home Healthcare Of Northern California Inc is seeking a part-time Occupational Therapist to serve Placerville and surrounding areas. You will evaluate and treat patients in home settings, designing therapy plans to help regain independence and coordinating with the rehab team.

The role offers per-visit pay starting at $95.00, flexible scheduling, and mileage reimbursement. A CA OT license, TB test, CPR, and a valid driver’s license are required.

Qualifications

• Licensed OT in CA
• Minimum 1 year OT experience preferred
• Experience with OASIS and WellSky (Kinnser) strongly preferred
• Current TB test (within last 12 months) and CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Provide effective patient care using approved therapy techniques and procedures
• Evaluate patients’ condition, function level, and treatment goals
• Develop individualized treatment plans and update as needed
• Supervise COTAs and HHAs per care plan requirements
• Document treatment notes and progress in the patient’s chart daily
• Assist patients and families with home exercise programs and adaptive equipment needs
• Recommend and evaluate adaptive equipment for the home setting
• Coordinate total patient care with the healthcare team
• Complete discharge summaries and obtain orders when needed
• Submit patient documentation and visit records in a timely manner
