Job summary

Beyond Home Health Care in New Jersey is seeking a licensed Occupational Therapist to provide in-home and community-based OT services to adults with developmental disabilities. The OT will evaluate functional performance, develop person-centered treatment plans, and deliver interventions to maximize independence in ADLs, IADLs, sensory processing, and community participation.

Qualifications

• Current New Jersey license as an Occupational Therapist.
• NBCOT certification preferred; required for new graduates.
• ACOTE-accredited OT program graduate.
• CPR/BLS certification current.
• Background check and fingerprinting per NJ DDD requirements.

Responsibilities

• Conduct comprehensive in-home and community-based evaluations and develop individualized treatment plans.
• Deliver one-on-one therapy services, aligned with ISP and person-centered goals.
• Document services per DDD standards, including notes and outcomes.
• Collaborate with Support Coordinators, families, caregivers, and the team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce goals between sessions.
• Maintain billing records consistent with DDD rates and authorizations.
• Complete required DDD trainings (Pre-Service, Danielle's/Stephen Komninos' Laws, etc.).
