Adventist Health is seeking an Occupational Therapist (OT) to join our home health and hospice teams serving California, Hawaii, Oregon and Washington. The role focuses on planning and conducting segments of treatment programs to restore and improve physical, social and mental functions, while collaborating with an interdisciplinary team.

Responsibilities include evaluating patients, developing individualized treatment plans, delivering therapy interventions, and ensuring accurate documentation.

Qualifications

• Master's degree in Occupational Therapy (OT) or Doctor of OT (OTD) required; license date criteria apply.
• OT licensure in all applicable states required.
• CPR or BLS certification required.
• Valid driver's license and minimum age 21 required.

Responsabilités

• Completes evaluations and addresses areas needing attention.
• Establishes individualized treatment and discharge plans; communicates with involved parties.
• Provides therapy interventions per approved protocols.
• Completes comprehensive clinical documentation within deadlines.
• Supervises assigned assistants.
• Performs other job-related duties as assigned.
