Job summary

Metro Physical & Aquatic Therapy is seeking a motivated Occupational Therapist to provide in-home, per diem care across Central Suffolk County. You will deliver 1:1 treatment, evaluate patients, and develop individualized plans per physician orders.

All levels welcome; mentorship available for documentation and compliance. Flexible schedule, opportunities for growth, and a supportive team environment are offered; 1099/W-2 options may apply.

Qualifications

• Licensed NY OT or eligible.
• OT program graduate.
• Home health experience preferred but mentorship provided for documentation and compliance.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards and the prescribed plan of care.
• Educate family members and home health aides on proper therapy techniques.
• Carry out occupational therapy programs to maintain or restore function and prevent dysfunction.
• Perform initial exam and document findings prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate treatment effects and adjust plans to maximize benefit.
• Willingness to treat geriatric patients.
