Job summary

Giving Home Health Care in Santa Fe, NM seeks a compassionate RN or LPN to administer patient care per physician plans in patients' homes. This 1099 contract role involves 32 hours monthly, with availability on weekends and mid-late mornings.

You will collaborate with CNAs, HHA, case managers, and patients’ physicians to provide one-on-one care. Responsibilities include executing care plans, documenting care, administering medications, assessing patients, and facilitating education for patients

Qualifications

• Graduate of an accredited college or university with nursing degree.
• Current State or Compact License as an RN or LPN.
• Current CPR certification.
• TB test and physical examination.
• Maintain required insurance.
• Good interpersonal skills.
• Ability to lift up to 50 lbs and assist with mobility.

Responsibilities

• Execute physician prescribed plans of care.
• Document care provided in the system of record.
• Administer prescribed medications and therapies.
• Conduct patient assessments and coordinate care.
• Assist with daily living activities.
• Monitor vitals and intake/output.
• Assess skin integrity and provide wound care.
• Monitor changes in patient condition and communicate with care team.
• Provide education and training to patient and family.
• Supervise CNAs and PAs in home settings.
