Job summary

Signify Health is hiring a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations, engaging with members in their homes and bridging gaps in care. You’ll use an iPad with a streamlined workflow and deliver preventive services without prescribing or altering treatments.

You must hold an active unrestricted license, board certification, and be willing to travel locally and to licensed states.

• Board certification (required).
• Multi-state licenses (preferred).

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

Job Highlights

• The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and optional diagnostic tests such as diabetic eye exams and spirometry.
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Clinicians are not required to prescribe medicine, order lab tests, or alter current treatment regimens.

A Note On Our Work Environment

As an in-home clinician, your primary workplace is the private residence of health plan members. You must be able to comfortably and safely perform duties in homes that may contain pets, secondhand smoke, and other airborne allergens or irritants.

Pay Range

$37.66 - $81.11 per hour. This range represents the base hourly rate or annual full-time salary for the job grade within which this position falls. The actual base salary will depend on experience, education, geography and other relevant factors.
