Job summary

Signify Health, part of CVS Health, seeks a part-time Clinician (NP or PA) to perform in-home health evaluations. Visits typically last about 45 minutes and include history review, physical assessment, and optional diagnostic tests, with no requirement to prescribe or alter current treatment.

Qualifications

• Active, unrestricted license in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations for health plan members.
• Engage with patients to review medical history, vitals and related information.
• Utilize iPad and clinical workflow; conduct assessments and document findings.
• Travel locally to visits and perform virtual visits when needed.
