Job summary

Signify Health, part of CVS Health, seeks an in-home clinician (NP or PA) to perform comprehensive in-home health evaluations. You will connect with patients in their homes, review medications and medical history, perform physical assessments, and coordinate with health plans for required tests.

The role emphasizes flexibility to travel locally and within licensed states and virtual visits when needed. The position offers full-time hours (40 per week), a broad benefits package, and a pay range

Qualifications

• Active, unrestricted license in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.
