Job summary

Signify Health, part of CVS Health®, is hiring a Full Time Clinician (NP or PA) to perform In‑Home Health Evaluations. You’ll assess patients in their homes and help bridge gaps in care through a compassionate, patient‑centered approach.

The role requires an active license, board certification, and willingness to travel within licensed states. You’ll use an iPad and clinical workflow to complete evaluations and coordinate with plans for quality care.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Join a national network of clinicians dedicated to improving health outcomes.
• Visit millions of people nationwide to connect them with the right care for their needs.
• Collaborate with Signify Health as a clinician in the specified role.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide diagnostic and preventive services (spirometry, DEE, bone density screening, etc.).
