Qualifications

• Master’s Degree or commensurate NP experience with license in good standing.
• Active State NP license and National board certification.
• Current BLS certification required.

Responsibilities

• Travel to patients' homes for HRAs and non-invasive assessments.
• Review medical history and document findings promptly.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic wellness education and preventative guidance.
• Maintain accurate, timely clinical documentation.
