Source One Management Services, LLC is seeking Nurse Practitioners in the Lowell, MA area for in-home health risk assessments. This role provides total autonomy and will start immediately, conducting HRAs for Medicare Advantage members without the need for prescribing care.

Qualifications

• Active NP license required with accepted specialties excluding Pediatrics, PMHNP, and Women's Health.
• Must have reliable transportation to travel across multiple counties.
• Ready to start ASAP with available hours exceeding 40/month.

Responsibilities

• Conduct in-home HRAs including medical history and evaluations.
• Provide education on preventive care.
• Document visits using a company-provided iPad.
