Job summary

Collage Home and Community Rehabilitation Partners is seeking an Occupational Therapist to provide patient-centered care in Ontario, CA and surrounding areas on a part-time basis. Therapists will work in patients' homes and communities, developing functional goals and treatment plans.

The role emphasizes evidence-based practice, collaboration with interdisciplinary teams, and durable outcomes. Licensure in California and a Bachelor's in OT are required; a master's degree is preferred.

Qualifications

• Bachelor's degree in OT and state licensure required
• Master's degree or professional certification preferred
• Experience with neurologically related disorders strongly preferred
• Excellent planning, organization, writing and time management skills required
• Ability to communicate with families, professionals, and co-workers

Responsibilities

• Evaluation and treatment of impairments including upper extremity function and ADLs
• Address cognitive needs and adaptive equipment requirements
• Collaborate with an interdisciplinary team to create individualized home-based treatment plans
• Deliver patient-centered OT services in patients' homes and communities
