Job summary

Collage Home and Community Rehabilitation Partners in Dallas, TX seeks an experienced Licensed Professional Counselor (LPC) to provide patient-centered care in patients' homes and communities in the Dallas area, with PRN positions available for clinicians who can accept patients for the duration of their stay.

Qualifications

• Completion of Master's degree in Psychology, Counseling or Social Work (LCSW) and state licensure required
• Excellent planning, organization, writing and time management skills are required
• Ability to communicate with families, professionals, and co-workers is a must

Responsibilities

• Areas of responsibility include but not limited to: Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
