Job summary

Collage Home and Community Rehabilitation Partners seeks experienced CNAs to deliver patient-centered in-home care in San Antonio, TX and surrounding areas. PRN positions available, one-on-one care in patients' homes, following individualized treatment plans and safety protocols.

Responsibilities include personal hygiene, mobility assistance, meals, medication reminders, light housekeeping, and reporting changes to the interdisciplinary team.

Qualifications

• Experience in healthcare, in-home care, or hospitality preferred.
• Ability to lift up to 50 pounds and assist with physical tasks.
• Valid driver’s license and reliable transportation required.
• Willingness to work nights and weekends in a PRN role.
• Demonstrated compassion and supportive communication.

Responsibilities

• Provide personal hygiene assistance and daily living support.
• Assist with medication reminders and mobility around home.
• Support activities of daily living and meal preparation.
• Perform light housekeeping and accompany clients to appointments.
• Report changes to the care plan to the interdisciplinary team.
