Collage Home & Community Rehabilitation Partners provides in-home neurological rehabilitation across Riverside, CA and surrounding areas. We are seeking experienced Physical Therapists for part-time positions to deliver truly patient-centered care in patients’ homes and communities.

Our interdisciplinary teams create integrated treatment plans, emphasizing evidence-based practice and functional goals to ignite patient engagement and durable outcomes.

Qualifications

• Bachelor's degree in Physical Therapy with state licensure and neuro rehab experience.
• Excellent planning, organization, writing, and time management skills are required.
• Ability to communicate with families, professionals, and co-workers is a must.

• Evaluation and treatment of impairments including lower extremity function, mobility, vestibular skills, adaptive equipment needs, environmental safety needs
