Job summary

Senior Care Solutions Group is seeking a compassionate MSW to join our Home Health team in Boca Raton on a PRN basis. You will provide in-home support, helping patients and families navigate emotional, social, and environmental challenges related to illness and aging.

You will work with an interdisciplinary team to promote independence, coordinate resources, and ensure high-quality care in the home setting. The role requires FL licensure and a minimum of one year of clinical experience.

Qualifications

• Master of Social Work (MSW) and FL licensure.
• Minimum 1 year of clinical experience; home health or healthcare setting preferred.
• Knowledge of community resources and care coordination.
• Proficiency with electronic medical records (Kinnser WellSky) preferred.
• Strong communication and problem-solving skills.
• Reliable transportation and valid driver's license.

Responsibilities

• Conduct initial assessments of emotional, social, and environmental needs.
• Develop and implement individualized care plans with the team.
• Provide supportive counseling to patients and caregivers.
• Assist with accessing community resources and programs.
• Participate in discharge planning and care coordination.
• Document services accurately in WellSky.
• Maintain compliance with regulations and standards.
• Collaborate with nurses, therapists, physicians, and the care team.
