Job summary

Addus Family company is seeking a compassionate Licensed Practical Nurse to provide direct in-home care to elderly, chronically ill, and disabled clients. The LPN will perform assessments, monitor vital signs, provide wound care, administer medications, and support care plan implementation under RN and physician direction.

You’ll educate patients and families, document in EHR, and coordinate with a mission-driven, patient-focused team.

Qualifications

• Active LPN license in the state where care is provided.
• Experience delivering in-home, patient-centered care preferred.
• Ability to perform assessments and monitor patient status.

Responsibilities

• Provide direct in-home care to elderly, chronically ill and disabled clients.
• Perform assessments and monitor vital signs.
• Provide wound care and administer medications as directed.
