Job summary

Adara Home Health, Inc. is seeking a Licensed Practical Nurse to provide in-home client care across Buffalo, Wayzata and surrounding areas. The LPN will travel to client homes, perform assigned nursing duties and coordinate with a Case Manager to update care plans.

The role requires MN LPN licensure, at least 1 year of healthcare experience, strong communication skills and reliable transportation. On-call rotation applies, with full-time hours Monday through Friday.

Qualifications

• Licensed Practical Nurse with MN license is required.
• Minimum 1 year healthcare experience preferred.
• Strong interpersonal and teaching abilities are essential.
• Excellent written and oral communication skills.
• Access to a dependable vehicle or reliable transportation.

Responsibilities

• Plan, provide and evaluate nursing care per policies and Case Manager guidance.
• Identify and report changes in client condition promptly.
• Document observations and teaching clearly and accurately.
• Administer medications and treatments as ordered and within legal limits.
• Coordinate with the interdisciplinary team and participate in client conferences.
• Maintain confidential client records and ensure timely documentation.
