Job summary

Addus Family Company is seeking a Licensed Practical Nurse to deliver compassionate, in-home nursing care to seniors and individuals with disabilities. In this role, you’ll provide skilled nursing services, monitor patient conditions, administer medications, and follow care plans under RN or physician direction.

You’ll educate clients and families, document care accurately, and coordinate with an interdisciplinary team.

Qualifications

• Valid LPN license required.
• Experience in in-home care or home health is preferred.
• Ability to follow care plans under supervision.
