Job summary

Modani Care is seeking a compassionate Male LPN to join our in-home care team serving seniors and adults in Georgia. The LPN will deliver individualized nursing care, collaborate with the care team to create and adjust care plans, and support clients in their home environment.

The role requires a current Georgia LPN license, ability to work days/evenings/weekends, a valid driver’s license, and strong communication skills.

Qualifications

• Current LPN license in Georgia (GA) is required.
• Ability to work varied shifts, including days, evenings, and weekends.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide high-quality, individualized care to clients, including medication administration, wound care, and other nursing tasks.
• Develop and implement personalized care plans with the care team.

• Communicate effectively with clients, families, and other healthcare professionals to ensure seamless care coordination.
• Maintain accurate and detailed records of client care and services provided.
• Participate in ongoing education and training to stay current with industry developments and best practices.
