Job summary

Rosaires Care is seeking an in-home Licensed Practical Nurse (LPN) to provide skilled nursing services to elderly, chronically ill, and disabled clients in their homes. You will monitor vital signs, administer prescribed medications, and follow individualized care plans to support safe, patient-centered outcomes.

You will collaborate with RNs, physicians, and family members to ensure continuity of care, maintain documentation, and ensure compliance with safety standards.

Qualifications

• Current LPN license and ability to provide in-home skilled nursing services
• Strong clinical assessment, medication/treatment administration, and vital sign monitoring skills
• Compassionate, patient-centered approach with clear communication and teamwork
• Ability to follow care plans and adhere to safety and compliance requirements

Responsibilities

• Deliver clinical services and maintain accurate documentation of care and observations
• Monitor and assess chronic conditions, reporting changes to the care team
• Administer medications and treatments according to provider orders and safety protocols
• Coordinate with RNs, physicians, caregivers, and family members to ensure continuity of care
• Ensure care meets compliance standards and client-specific needs
