Job summary

Phoebe Putney Health System is seeking a Licensed Practical Nurse (LPN) for home health care. The LPN will provide one-on-one skilled nursing care, assess patient needs, and develop a care plan with the physician for patients in their home.

Responsibilities include monitoring patient progress, documenting outcomes, and maintaining confidentiality. A Georgia license, BLS, and clean driving record are required; ACLS is preferred.

Qualifications

• Vocational / Technical Degree in Graduate of an accredited school of Practical Nursing. (Required)
• 1 - 2 years Practical Nursing Experience (Preferred)
• Licensed Practical Nurse (LPN) with current Georgia license, Basic Life Support (BLS)
• Advanced Cardiac Life Support (ACLS) preferred
• Must possess and maintain a valid Georgia Driver's License.
