Job summary

Collage Home and Community Rehabilitation Partners is seeking experienced Licensed Clinical Social Workers to provide patient-centered care in San Gabriel, CA and surrounding communities, with PRN availability for ongoing patient stays.

You'll join interdisciplinary teams delivering home-based rehabilitation for neurological and complex orthopedic conditions, with an evidence-based approach and strong emphasis on ethical, honest care in the client's own environment.

Qualifications

• Master's degree in Social Work completed and valid LCSW licensure.
• Excellent planning, organization, writing and time management skills.
• Ability to communicate with families, professionals and co-workers.

Responsibilities

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans.
