Job summary

Accredo Specialty Pharmacy, part of Evernorth Health Services, seeks a Home Infusion Registered Nurse to deliver care in patients' homes, coordinating with care teams and documenting progress.

The role requires an active RN license, 1 year in critical care/acute/home health, and IV infusion proficiency. Travel within a large geographic region is required, with some evenings/weekends as needed.

Qualifications

• Active RN license in the state of practice.
• Minimum 1 year in critical care, acute care, or home health care.
• Proficiency in IV insertion and infusion techniques.
• Valid driver's license and ability to travel within a large geographic region.
• Ability to perform multiple patient visits per week including some evenings and weekends.

Responsibilities

• Provide safe administration of specialty medications (including IV infusion) in patients' homes.
• Partner with pharmacists and care teams to ensure holistic patient well-being.
• Document assessments, treatments, and progress in patient records.
• Serve as primary contact for patient updates and care coordination.
• Demonstrate autonomy in clinical decision-making to achieve optimal outcomes.
