Job summary

Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members around Greenwood, IN. This role offers flexibility and travel with a focus on community care.

Nurse Practitioners conduct comprehensive non-invasive assessments, review medical history, and provide basic wellness education in patients' homes, without prescribing in the home. A state NP license and board certification are required.

Qualifications

• Master's degree in nursing (NP) required or commensurate experience.
• Active NP license in the state.
• National board certification.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred but not required.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
