Pathways Healthcare, a leading home health and hospice organization, seeks a Chaplain to provide spiritual care to patients and families in the hospice program. You will assess spiritual needs, develop individualized plans of care, and collaborate with an interdisciplinary team to ensure compassionate, patient-centered support.

The role involves facilitating meaningful services, liaising with community clergy, and documenting care for quality improvement.

Qualifications

Graduate of an accredited seminary or school of theology with experience in death and dying.
Ability to work as a member of an interdisciplinary group.
Knowledge of hospice philosophy of care.
Willingness to network with community clergy and congregations.

Responsibilities

Assures spiritual assessment of patients and families in the hospice program and ensures timely services.
Facilitates development of the individualized plan of care via comprehensive assessment.
Provides direct spiritual care to patients and families.
Serves as liaison to community chaplains and spiritual counselors.
Maintains records of spiritual care services for quality assurance and program review.
Attends patient care conferences and interdisciplinary meetings.
Provides consultation, education, and support to the interdisciplinary team.
