Job summary

Inspired Home Care is seeking a Part Time/PRN Home Health Nurse to manage a personal caseload of patients in a one-on-one home setting. You will perform visits to monitor condition, vital signs, wound care, and IV therapy, while ensuring patient comfort and safety.

You will educate families on care strategies, coordinate with therapists and physicians, and suggest tools to improve quality of life. Strong communication and travel willingness are required.

Qualifications

• State RN license required.
• 1 year of experience in in-home care.
• Excellent communication and time management skills.
• Compassionate and friendly demeanor.
• Ability to travel to multiple patients per day.
• Preferred knowledge of WellSky Kinnser.

Responsibilities

• Assess patients' condition during every visit and chart observations.
• Perform evaluation tasks including vital signs, medication review, Oasis assessment, wound care and IV therapy.
• Dress or re-dress open wounds and assess healing progress.
• Educate patients and their families on home health care strategies and procedures.
• Coordinate with Occupational Therapists, Physical Therapists, Speech Therapists, Physicians and others involved in the care plan.
• Recommend devices or tools that might improve patient quality of life.
• Listen to patient concerns and respond to requests.
