Job summary

Signify Health is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. You will visit members in their homes, review medications and history, perform physical exams, and, when ordered, run diagnostic tests using portable tools and an iPad workflow.

The role is part-time/per diem with an hourly base rate plus incentives, malpractice coverage, and supplies.

Qualifications

• Active, unrestricted licenses in coverage area(s).
• Board certification required. Multi-state licenses preferred.
• Experience delivering patient care in home or community settings is a plus.

Responsibilities

• Provide in-home health evaluations for health plan members.
• Bridge gaps in care by connecting members to the right services.
• Engage with members for health questions and education.
• Complete assessments using an iPad workflow and clinical guidelines.
• Travel locally within licensed states and conduct virtual visits as needed.
