Job summary

Signify Health is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations, visiting members at their homes across communities in Massachusetts. You will conduct health histories, physicals, and medication reviews, and may perform optional diagnostic tests as ordered by health plans.

The role supports flexible scheduling, includes a typical 29.5 hours per week, and requires active licensure in MA with board certification.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
• Able to cover Worcester, MA primary locations.

Responsibilities

• Provide in-home health evaluations with a 45-minute visit.
• Engage with patients and collect health data and history.
• Perform physical evaluations and review medications and plans.
• Conduct virtual visits as needed and support diagnostic services.
