Job summary

Qualifications

• Must have current nursing license in practicing state.
• Arizona Fingerprint Clearance Card required.
• Valid driver's license with auto insurance and reliable transportation.
• Current CPR certification.
• Two years' experience as LPN/LVN in a clinical setting.

Responsibilities

• Provide skilled nursing care to patients, working onsite in their homes under the direction of an RN.
• Implement individualized care plans in collaboration with patient, family, and healthcare team.
• Educate patients and families on disease management, treatment options and self-care techniques.
• Maintain accurate records of patient care, including visits, interventions and outcomes.
• Maintain contact with patients, physicians, clinical managers, and other team members; participate in care coordination and discharge planning.
