Job summary

Lucet at Home seeks In‑Home Assessment Providers (NPs or PAs) to deliver comprehensive health risk assessments in patients’ homes across assigned markets. You’ll document findings using templated workflows and coordinate with the care team to support whole-person health.

Strong clinical assessment skills and EMR proficiency are essential. A valid state license, board certification, and reliable transportation are required for this field-based role.

Qualifications

• Active, unrestricted NP or PA license in the assessment state.
• Board certification (ANCC, AANP, or NCCPA) is required.
• Current BLS certification is required.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform in-home health risk assessments for Medicare, Medicaid, and ACA members.
• Use pre-populated templates for documentation.
• Submit chart notes within required timeframes.
• Maintain clear communication with clinical leadership and operations teams.
