Job summary

Qualifications

• Licensed Physical Therapist in Ohio (OT/PT license as applicable).
• Experience delivering geriatric/home-based PT preferred.
• Ability to develop and adjust plans of care based on evaluations.

Responsibilities

• Provide one-on-one PT to older adults in-home or in senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and function.
• Document effectively in FOX's EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
