Job summary

FOX Rehabilitation in Durham, NC seeks licensed Physical Therapists to deliver one-on-one geriatric care in patients’ homes and senior living communities. You’ll develop personalized treatment plans, perform evaluations, and support mobility and independence within the Geriatric House Calls™ model.

Clinicians may incorporate telehealth and other virtual care to extend access, improve continuity of care, and support more patients.

Qualifications

• Valid Physical Therapy license in the state of practice or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences and senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support wellness, longevity, mobility, and independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility
