Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care in homes and senior living communities across Northwood, IA and surrounding areas. You will improve mobility and independence while building lasting patient relationships and managing your own schedule within the Geriatric House Calls™ model.

Patients include private residences and senior living settings; telehealth/virtual care may be incorporated for flexibility and continuity of care.

Qualifications

• Active Physical Therapy license in the state of practice.
• Degree from an accredited Physical Therapy program (DPT).
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Deliver therapy sessions to improve mobility, longevity, and functional independence.
• Document effectively within the EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists as part of an interdisciplinary team.
• Offer telehealth/virtual care when appropriate to extend access.
