Job summary

FOX Rehabilitation, an in-home and senior living care provider, seeks licensed Physical Therapists to deliver one-on-one therapy across private residences and communities in Massachusetts.

You will evaluate patients, create individualized plans, and provide evidence-based treatment to improve mobility and independence. Telehealth options are available to extend access and flexibility within a collaborative, multidisciplinary team.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home and community settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver sessions that support wellness, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth or virtual care when appropriate to extend access.
