Job summary

Herewith is seeking a compassionate Care Partner to support a patient transitioning to home dialysis in Peoria, AZ. This role provides one-on-one care, not an agency rotation, helping the patient feel safe and supported as they start home-based treatments.

Training covers patient-specific dialysis procedures, home setup, and ongoing clinical team support, ensuring you feel confident. You will assist with setup, maintain a clean environment, provide companionship, and monitor safety.

Qualifications

• 1+ year dialysis experience or healthcare background (CNA/MA/caregiver)
• Familiarity with home dialysis systems preferred
• Willing to complete provided training for home dialysis
• Steady, supportive, and patient during adjustment period
• Strong communicator who works well with a clinical team
• Must pass Herewith background check

Responsibilities

• Assist with dialysis treatment setup (HomeChoice Claria APD or NxStage dialysis machine)
• Keep the treatment environment clean, organized, and safe
• Offer companionship and emotional reassurance
• Help the patient remain on schedule with treatment times
• Provide light ADL support as needed
• Monitor safety and report concerns to the coordinating nurse
• Maintain simple documentation and shift notes
