Herewith in Irvine, CA is seeking a compassionate Care Partner to support a patient transitioning to home dialysis. You will set up dialysis, maintain a clean environment, offer companionship, and coordinate with the clinic team to ensure timely treatments.

This role provides training for patient-specific dialysis processes and home setup, with a dedicated clinical team backing you. You will work independently with a single client, building trust and contributing to safe, patient-centered home

Qualifications

• 1+ year dialysis experience (home or clinic)
• Familiarity with home dialysis systems (NxStage & Tablo)
• Open to in-home dialysis support and willing to complete provided training
• Steady, supportive, and patient — especially during an adjustment period
• Strong communicator who works well with a clinical team
• Must pass a Herewith background check

About This Role

We are seeking a compassionate and reliable Care Partner who is open to supporting in-home dialysis for a patient who currently receives dialysis in a clinic and will transition receiving treatments at home. This role provides consistent, one-on-one support, not an agency rotation - allowing you to build rapport and become a trusted part of her weekly care routine. Training will be provided, including patient-specific dialysis processes, home setup guidance, and ongoing support from our clinical team to ensure you feel fully confident. This position is ideal for someone who values patient dignity, understands the unique challenges of dialysis, and wants to make a meaningful impact during a major care transition.
