Job summary

Herewith in Phoenix, AZ is seeking a compassionate Care Partner to support a patient transitioning to home dialysis. This role offers $25 per hour, four days a week, ASAP start, with training provided by a clinical team.

You will assist with dialysis setup on NxStage or Tablo, keep the environment safe, offer companionship, help with scheduling, provide light ADL support, and document shift notes. Experience with dialysis and willingness to learn are required; you will join a dedicated care

Qualifications

• 1+ year of dialysis experience (home or clinic).
• Familiarity with home dialysis systems preferred (NxStage, Tablo).
• Willing to complete the provided training.
• Strong communicator and reliable team member.

Responsibilities

• Assist with dialysis treatment setup (NxStage or Tablo).
• Keep the treatment environment clean and safe.
• Provide companionship and emotional support.
• Help maintain schedule and provide light ADL support.
• Monitor safety and report concerns to the coordinating nurse.
• Maintain simple shift notes and documentation.
