Job summary

SYNERGY HomeCare of North Valley seeks a compassionate Day Shift Caregiver to join our Paradise Valley, Arizona team. You will provide personal care, assist with daily living activities, and promote independence for our clients in their homes.

Responsibilities include medication management, record-keeping, outings with clients, and collaboration with healthcare professionals to create individualized care plans; previous caregiving experience is preferred, and a high school diploma is required.

Qualifications

• High school diploma or equivalent required.
• Previous caregiving or related field experience preferred.
• Ability to lift up to 50 pounds and transfer clients as needed.
• Valid driver's license and reliable transportation (if applicable).

Responsibilities

• Providing personal care and daily living assistance to clients in their homes.
• Assisting with medication management and administering medications as needed.
• Accompanying clients on outings and participating in activities that promote socialization and engagement.
• Maintaining accurate records of client care and communicating with healthcare teams.
• Collaborating with colleagues and healthcare professionals to develop and implement individualized care plans.
