Responsibilities

• Provide in-home visits for the Virtual Care at Home program under Medical Director supervision.
• Perform medication administration, IV setup, phlebotomy, dressing changes, and vital signs checks.
• Educate patients and families about disease state, condition, and discharge plan.
• Coordinate discharge appointments, medical supply needs, medications, and referrals with the care team.
• Document patient conditions and report drug or treatment reactions to the provider and care team.
