Responsibilities

• Conduct clinical assessments and formulate treatment plans with consumer and family.
• Provide individual, group, play and family therapy in-home and community settings.
• Deliver outreach services to consumers and families per FIND standards.
• Collaborate with staff and outside sources to coordinate care.
• Document assessments, treatment goals and services in a timely manner.
