Job summary

OneWell Health Care in Bluffton, SC is seeking an In-Home Caregiver to provide compassionate, non-medical support in clients' homes. You will assist with daily living activities, companionship, light housekeeping, and meal preparation while following care plans. You will document care activities, monitor changes, and communicate with a supervising RN and family. A valid driver’s license and reliable transportation are preferred.

Qualifications

• High School Diploma or GED required.
• Prior caregiver/home health aide experience preferred.
• Experience supporting elderly or individuals with disabilities is highly desirable.
• Ability to follow care plans and written instructions.

Responsibilities

• Provide daily non-clinical personal care and assistance with ADLs.
• Support health and wellness routines and safe transfers.
• Document care logs and report changes to the RN Case Manager.
• Assist with meals, light housekeeping, and transportation as needed.
• Maintain confidentiality and adhere to HIPAA.
