WelbeHealth is seeking Home Care Assistants to deliver personal care and support to participants in their homes, guided by the IDT. You will work a full 8-hour day, visiting 2–5 patients, with travel time and mileage reimbursements.
Responsibilities include assisting with ADLs and Instrumental ADLs, recording vital signs, and supporting the Rehabilitation team with home exercise programs. Bilingual applicants are preferred, and certification requirements apply.

Our Home Care Assistants are a vital part of our team, providing aid to our participants in their homes as well as contributing to the care planning process as a member of our interdisciplinary team (IDT). Reporting to the Home Care Supervisor, the Home Care Assistant will provide safe delivery of personal care services to WelbeHealth participants in their home, as scheduled and directed.
