Job description

Mentor Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities.

Job Summary

Part‑time only! Provide one‑on‑one care to adults who live with family or other caregivers or independently, helping them succeed in their home environment and participate fully in community life. Work within home‑based community settings of the IHSS program and collaborate with our HCBS team to ensure health and safety.

Location & Pay
  • West Valley locations: Glendale, Peoria, Phoenix, Avondale, Goodyear, Surprise
  • Pay Rate: $15.15 per hour
Position Type & Scope
  • Non‑exempt, hourly role; full‑time or part‑time
  • Scope: Provide direct support to 4‑6 individuals in residential settings or up to 10 in periodic programs
Key Responsibilities
  • Deliver support per individual service and program plans, including socialization, health maintenance, medication administration, and skill/behavioral development.
  • Assist with daily activities: personal care, housekeeping, recreation, transportation, community integration, shopping, citizenship, and other activities of daily living.
Core Functions & Services
  • Direct Support: assist with socialization, behavioral development, personal care, housekeeping, recreation, transportation, community orientation, shopping, financial management, citizenship, and other daily living activities.
  • Records: accurately document progress and activity; review records and logs; maintain confidentiality.
  • Management of Individuals’ Assets: make cost‑effective purchases within budget and according to user preferences.
  • Relationships: maintain professional relationships with individuals, families, guardians, and case managers; greet family members and visitors; implement Sevita Customer Service Standards.
  • Reporting: report any alleged abuse, neglect, medical or behavioral incidents in accordance with internal and external standards.
  • Rights: maintain confidentiality, respect the rights of persons served, uphold the bill of rights, practice universal precautions, and assist individuals in exercising their rights.
Health Care & Meal Preparation
  • Appointments: accompany individuals to medical appointments and relay orders and information.
  • Medications: accurately administer or support self‑administration, document deliveries, report errors, and secure controlled medications.
  • General Health Care: monitor health, document concerns, communicate with nurses or supervisors, call 911 in emergencies, and follow health directives.
  • Medical Supplies and Equipment: follow procedures for using equipment and report malfunctions.
  • Meal Preparation: prepare or support preparation of meals in accordance with menus and individual choices, including special diets.
Employment Responsibilities
  • Training: attend orientation and ongoing training; participate in monthly staff meetings.
  • Employee Scheduling: work scheduled hours, arrive on time, stay until replacement arrives, notify supervisor of absence promptly, and fill in for shifts as needed without causing unapproved overtime.
  • Teamwork: work collaboratively, value contributions, be courteous, communicate openly, listen, and show respect.
  • Workplace Safety: comply with safety policies, report hazards, and participate in safety training or activities.
Maintenance & Safety
  • Vehicles: transport individuals safely, use seat belts and equipment, and report accidents promptly.
  • Housekeeping: perform daily and seasonal housekeeping, maintain a clean and safe environment, and report hazards.
  • Maintenance and Repair: monitor safety, perform minor repairs, yard maintenance, and report needed equipment or facility repairs.
  • Safety: check water temperature as required, participate in safety drills, and protect individuals during emergencies.
Other Duties
  • Perform other duties and activities as required.
  • MPA = Mobile Punch Authorized for timekeeping.

Minimum Knowledge and Skills Required

The following requirements are representative of the knowledge, skill, and/or abilities required to perform the job.

Education and Experience
  • High school diploma or equivalent preferred
  • Six months of experience in human services preferred
  • Must be 18 years of age
  • Working knowledge of computers
Certificates, Licenses, and Registrations
  • Valid driver’s license in good standing
  • Car registration and insurance if providing transportation
  • All state‑required training(s) and certification(s) completed on time
  • Maintain a valid driver’s license, meet insurance requirements, and be approved to drive; otherwise a Non‑Driver form must be signed and driving restricted until MVR requirements are satisfied
Work Environment
  • Works at the program location and may accompany individuals into the community
Physical Requirements
  • Heavy work; exert up to 100 pounds occasionally, up to 50 pounds frequently, and up to 20 pounds constantly to move objects

  • Employees must cooperate with the licensee and Department of Human Services staff in any inspection, inquiry, or investigation.
  • Must have a valid driver’s license in good standing in the State of New Jersey; if commuting from another state, must have a valid license in that state.
  • At minimum, a high school diploma or equivalent is required.
  • All employees hired after 6/15/2018 must submit to drug testing before employment and randomly thereafter; employees must submit to criminal background checks and be cleared by the Department of Human Services.
  • All employees must complete and demonstrate proficiency in all required trainings.
  • Shall be at least 18 years of age.
