Job summary

NuChoice Health Partners seeks a dedicated Personal Care Assistant to join our home care team in Hermitage, PA, delivering one-on-one support in patients' homes to promote independence, comfort, and well-being.

You will assist with daily living activities, meal preparation, medication reminders, and companionship, while maintaining accurate records and collaborating with healthcare professionals in a fast-paced, flexible environment.

Qualifications

• Experience in home healthcare or caregiver role.
• Ability to work independently in patients' homes.
• Excellent communication and empathy in patient care.

Responsibilities

• Assist patients with daily living activities (bathing, grooming, dressing).
• Prepare and cook meals to support nutrition and hydration.
• Provide medication reminders and monitor adherence.
• Offer companionship and emotional support to patients.
• Assist with mobility exercises and maintain a safe home environment.
• Collaborate with healthcare professionals to ensure patient well-being.
• Document tasks and time in/out with accurate records.
