Job summary

American River Healthcare is seeking experienced Registered Nurses to provide shift-style in-home care to patients through their Advance Care at Home program. This role allows RNs to work 1:1 with patients, delivering hands-on care in a home setting. Ideal candidates will have an active RN license in California and Med-Surg experience preferred. Duties include managing patient care plans and working on a shift-style schedule, including every other weekend. Candidates must be independent, adaptable, and possess strong communication skills.

Qualifications

• Must have an active RN license in California.
• 1–2+ years of Med-Surg experience preferred.
• Experience in home health or hospice is a plus.
• Comfortable working independently in the field.

Responsibilities

• Provide routine and PRN nursing visits in a home setting.
• Deliver hands-on care including vital signs, wound care, and IVs.
• Manage patient care plans and communicate changes.
• Work shift-style schedules including every other weekend.
• Use critical thinking and assessment skills daily.
• Build relationships with patients and families.
